Nathan Shively-Sanders da41217f43 Binding element with parent type any is any 
Previously if the binding element had an initializer, then that type
would be used. But this is incorrect:

```ts
function f(x: any) {
  let { d = 1 } = x;
  // d should have type any not number.
  // f can be called with anything:
}
f({ d: 0 });
f({ d: 'hi' });
f({});
```

1) Replace range, rather than node, to leave trivia intact.
2) Only replace node in the innermost scope - otherwise insert as usual
and delete the original statement.

Andrew Casey 3b4cbebee0 Handle repeated substitution of the same symbol during extraction 
Old: Insert the same Node for every occurrence.  The position was
repeatedly mutated during printing.
New: Thunk the Node so that a new one is constructed for every
occurrence.
