Check the qualified instead of the local name in the stack of open elements

Update parsing logic for <menuitem> and <menu> elements
...
The current spec (2016.05.15) is missing the instruction to "reconstruct
the active formatting elements" to match the handling of the <option>
element

Everybody else is doing it, so why can't we?
